Getting Started as a New Patient

Step 1: Choose the Most Convenient Location

Innovative Dermatology has two office locations to better serve our patients. Before scheduling, please review both locations and their office hours to decide which office is most convenient for you.

Please consider:

  • Distance from your home or work

  • Available appointment days and times

  • Your preferred physician or provider, if applicable

Step 2: Review Our Accepted Insurance Plans

Before your visit, please review our list of accepted insurance plans to confirm your coverage.

Even if your insurance is listed, we recommend that you:

  • Confirm that Innovative Dermatology or our Doctors are in-network with your specific plan

  • Check whether a referral is required

  • Verify your deductible, copay, or specialist visit benefits

This step can help avoid unexpected costs and make your visit smoother.

Step 3: Read Our HIPAA Privacy Policy

Your privacy is very important to us. Please review our HIPAA Privacy Policy (PDF) to understand how your medical information may be used and protected. Read it but you don’t have to print it.

After reviewing the policy, please complete and sign the HIPAA Acknowledgement Form (PDF). You can fill out directly on the form, then print and sign the form. Bring it with you to the appointment.

This form confirms that you have received or had access to our Notice of Privacy Practices.

Step 4: Review Our Financial Policy

Please read our Our Financial Policy carefully before your appointment. This policy explains important information about:

  • Copays

  • Deductibles

  • Coinsurance

  • Self-pay balances

  • Missed appointment or late cancellation policies

  • Payment expectations at the time of service

To save time at check-in, please complete your online patient registration OR optional paper form in PDF file before your appointment.

Your on-line registration may include:

  • Demographic information

  • Insurance information

  • Medical history

  • Medication list

  • Allergies

  • Pharmacy information

  • Emergency contact information

Once you have reviewed the necessary information and completed your registration, you are ready to schedule your visit.

  • Pathology reports or biopsy results, if available

  • Photos of skin flares or rashes if the condition comes and goes
